If I leave the county, how long will my insurance remain in
effect?
As an employee, your benefits will remain in effect until midnight
on the last day of the month in which you were employed.
Does my insurance plan pay for glasses or contact lenses?
Both Paramount Health Care and the Lucas County Health Plan
provide a vision hardware reimbursement of $100 every 24 months
for each enrolled member.
We are moving my mother into our home, as she is no longer
able to care for herself. We will be claiming her on our income
taxes. Is she eligible for health insurance through Lucas County
as a dependent of mine?
No. The Lucas County Employee Benefits Eligibility Rules
specifically state the definition of a covered dependent. Parents
are not eligible dependents for benefits through Lucas County.
I am currently going through a divorce. What do I need to do
to drop my spouse?
Once your divorce is final, complete and submit a Lucas County
Health Benefits Enrollment/Change Form dropping your spouse, along
with a copy of the signed, file stamped divorce decree.
